Digimon Survive is a story-based game where players' decisions, answers, and actions can greatly impact the narrative and the story's ending. The game follows a 3-way Karma system that depends on players' alignment, as shown by their choices, and dictates how the story ends.

How many endings does Digimon Survive have?
Digimon Survive has three main endings, one for each of the alignments (Moral, Wrathful, Harmony), and a True Ending that is tied to the Moral path. There's another ending, which is the bad ending, that technically makes a total of five endings.
How to reach the different endings in Digimon Survive
All endings (except the bad ending) are heavily based on the amount of Karma points that Takuma accumulates, which are based on his choices throughout a single playthrough.
For the three main endings, the story splits in Chapter 8, where Takuma and Miyuki are transported back to the human world. During this scene, you'll have the option to save the game before receiving additional Karma-related options. These choices are the Karma alignments in which you have the most points, and the last one is the "Never return" option. Not all karmic options may appear in this scene.
Choosing any of the Karma-related options will send you to the story line that has a specific ending based on the alignment of the answer you chose.
As for the True Ending, it is based on both Karma alignment and surviving characters in the final chapter, and it can only be achieved in a saved file in New Game+ or in a subsequent playthrough.
SPOILER WARNING: The details from here on will spoil the events that happen in all the game's endings.
Digimon Survive "Moral" Ending
This ending can be obtained by selecting the "Be true to my friends" option in the portal scene in Chapter 8. In this ending, the remaining characters will have to fight the Master in his egg form. Once defeated, the Master will return to his clean form.
The Master will then reveal that the reason behind his child sacrifices was to prevent the parallel world from collapsing and to protect the Digimon residing in it. But later, the children suggest that simply the acknowledgment and belief of the people from the Human World will prevent the Digital World from collapsing.
The children then decide to return to the Human world to spread word of the Digital World, but their Digimon partners decide to stay in the meantime, as they consider that the Human World may not be ready for them yet.
Digimon Survive "Harmony" Ending
This ending can be obtained by selecting the "Save the world" option in the portal scene in Chapter 8. In this ending, Miu will inevitably die, which will cause Kaito to form a pact with Piedmon to create Boltboutamon. The remaining characters will have to fight the Master after he consumes Boltboutamon. Once defeated, Boltboutamon will reappear in his weakened state.
The children will then realize that the portal connecting the two worlds is expanding and beginning to solidify, representing another threat that the two worlds will merge again. The children then acknowledge the fate of both worlds and decide to return to the Human World, bringing their partners with them.
As expected, the transition of cohabitation between both humans and the Kemonogami was difficult and caused discord between the two factions. However, the children eventually helped establish the Japanese Institute of Electronic Symbionts to help troubled children who gained a partner of their own. In this ending, they are still referred to only as Kemonogami or Electronic Symbionts, and not Digimon.

Digimon Survive "Wrathful" Ending
This ending can be obtained by selecting the "Stay with Agumon" option in the portal scene in Chapter 8. In this ending, Saki will die and Aoi merges with Labramon in her last breaths to create Plutomon. The Master then merges Plutomon into himself to become stronger. Once defeated, Aoi's soul reappears once more.
The children will then realize that the portal connecting the two worlds is getting bigger and can no longer be closed. This will lead the two worlds to merge again. The children then acknowledge the fate of both worlds and decide to return to the human world and bring their partners with them.
The merging of the two worlds caused chaos and destruction. Some children managed to form a bond with the Kemonogami and are therefore being avoided by other humans. Takuma and Kaito decide to take matters into their own hands and seek out these children to help them join the cause of keeping peace in the new world.
How to get the true ending in Digimon Survive
This ending can only be achieved by playing through a New Game+ save file. Another requirement to get this ending is to ensure that everyone survives until the end; this means that Ryo and Shuuji must survive.
There will be a better opportunity to increase affinity with Ryo, as the affinity points gained in conversations will receive a small boost in NG+. As for Shuuji, as long as Ryo survives, he will survive as well. Following the moral path will also ensure that all children survive.
The group will meet Haruchika who is a human who has united with Fanglongmon. Due to Haruchika being swallowed by malice, Fanglongmon entered his Ruin Mode. Later, Miyuki is possessed by her ancestor, Yukiha, who is Haruchika's sister. This weakens Fanglongmon and once defeated, he will return to his calm nature and release Haruchika. After the reunion, the children returned to the human world along with the Professor, Miyuki, Haru and their partners.
An epilogue follows as Takuma narrates that although the meeting between the Digimon and humans was difficult at first, they began to get along. The next scene shows three children near the shrine as they wait to see a monster, after which they meet DemiVeemon.
Digimon Survive "Bad" Ending
This ending is abrupt, as it ends in Chapter 8 if the option to never return to the Digital World has been chosen. The scene then shifts to show Takuma some time later, back at their house, while his mother reminds him of anything he might have forgotten.
They are preparing to leave their home as part of emergency relocation efforts, as the world is in chaos due to the strange phenomena that have plagued it since that fateful day.
The screen will then transition showing a Game Over and then revert to the main screen.
