The end of season five brought major losses for Tommy Shelby, including Barney Thompson, Aberama Gold, and his aunt Polly, played by Helen McCrory who passed away from cancer in 2021.
Betrayed by Billy Grade, who discovered Tommy's plans through Finn Shelby, who let his brother's plans slip, resulting in his expulsion from the family and Billy eventually being killed.
Tommy Shelby's Grand Plan
The protagonist ends up involved in a plot with Mosley, the IRA (Irish Republican Army), and American gangster Jack Nelson. Tommy allies himself with Alfie Solomons trying to ensure the distribution of his opium in the United States, mainly in Boston.
Foreseeing an ambush, Tommy sends Johnny Dogs to keep watch over Michael Gray and his associates. The young man moves the bomb that was meant for Tommy. This ends up killing much of the Boston mob, except for Michael who is directly confronted by the leader of the Peaky Blinders. With this, the Jewish mob will have more space in the city, and led by Solomons, will be a great partner for them.

Tommy is Diagnosed with a Disease
Midway through the season, we witness the tragic death of Ruby, Tommy's daughter. The protagonist is also diagnosed with a serious illness. With tuberculoma, an inoperable tumor, he will have little time to live, as the disease has no treatment.
With his remaining time, he organizes his business and ends up isolating himself in his final days. Alone on a mountain, Tommy has a vision of Ruby, who tells him he will not die. Next, he sees a newspaper clipping showing his doctor alongside Oswald Mosley and his second wife.
This confirms that his doctor was conspiring with the fascist, and so Tommy leaves his camp with his horse and heads back to his old life.
